Referral Pathway

The admission process is dependent on the age of your child.

Here's how it works:

  • For children 0-5 years of age, please see below.

  • For children 5-18 years of age, call Child and Adolescent Mental Health Intake at 780-342-2701.

Please Note: Certain CASA programs have their own referral pathway. Refer to the programs page for more specific information on each program's referral pathway. 

For children 0-5 years old

Children 0-5 years old are referred directly to CASA by the parents/guardians:

1. If your child is birth to 12 months of age, download and fill out the Infant Intake Form and send it by fax or mail to the address below.

2. If your child is 1 year to 4 years 9 months of age, download and fill out the Preschool Intake Form and send it by fax or mail to the address below.
3. If you do not have access to a printer and would like an Intake Form mailed to you, please call CASA at 780-400-4503

4. If you are a physician referring a child, please have the family fill out the intake form and fax or mail to the address below.

What happens when your child comes to CASA?

When you bring your child to CASA:


  • He or she is assessed by a mental  health therapist who specializes in working with infants and preschoolers. The therapist may be a nurse, a social worker, an occupational therapist, a psychologist or a psychiatrist..

  • A treatment plan is developed in collaboration with the family.

  • Families are encouraged to discuss their request for mental health services with their family doctor if they haven't already done so.

For children 5-18 years old

Children 5-18 years old are referred to CASA through the Alberta Health Services' Children, Youth and Families (CYF) Mental Health Intake Office.

What happens at Children's Mental Health Intake?

Regional Intake coordinates referrals to CASA and all other major public providers of children's mental health treatment services in the Edmonton Capital Region.

  • Upon calling, Intake staff members will complete a telephone interview questionnaire.

  • Following the screening, referrals are sent to various children's and adolescent's mental health providers, including CASA, based on the unique conditions of each child, including where they live, the child's age, the presenting concern and any history of treatment.

  • Referrals sent to CASA by CYF Mental Health Intake are distributed to the appropriate CASA program.

  • Families are encouraged to discuss their request for mental health services with their family doctor if they haven't already done so.
